- perform a thorough but efficient assessment of older adult’s capacity for medical-decision making.
- develop communication skills to effectively communicate with older adults with sensory impairment, including vision loss, hearing loss, or combination of the two.
- provide a comprehensive plan to screen, treat, and prevent delirium.
- identify risk factors that contribute to unsuccessful transitions of care.
- summarize the various care settings across which older adults receive acute and post-acute care.
